In our efforts to continue to keep PMHA as great as it is, we are asking that one parent/guardian take time to complete a survey about your child/children’s 2018-2019 hockey season. Your feedback through this survey is confidential and helps us make decisions and/or improvements for next year’s season. It is a short survey and will only take a few minutes of your time.
